This patch pushes dummy frames when loading code for the
profiling purpose.

The following methods push a dummy frame:
* `Kernel#require`
* `Kernel#load`
* `RubyVM::InstructionSequence.compile_file`
* `RubyVM::InstructionSequence.load_from_binary`
